3D Immediate Transformation of The Fully Edentulous Patient

Description:
Immediate loading of threaded dental implants with fixed restorations has been a documented treatment modality for more than 20 years. In that time, materials, surgical protocols, treatment methods and restorative options have evolved so that these procedures can be now be considered routine and predictable. The original protocols required an abundant number of implants in large volumes of dense bone, laborious provisional restorations and expensive precious metal based final prosthesis. Current methods permit treatment of patients with limited bone, streamlined provisionalization methods, and new materials and technology to provide predictable and efficient solutions. This presentation will provide an overview of some of the key principles required for successful immediate loading of fully edentulous patients. It will also demonstrate some of the newer concepts and materials and options for efficient and routine treatment.

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
After tooth extraction, bone resorption of varying amounts always occurs as the edentulous site of the alveolar process undergoes both qualitative and quantitative changes. Different techniques have been presented in literature in order to preserve the post-extraction site, including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials. Once hard tissue has been successfully augmented, an adequate soft tissue thickness becomes mandatory in order to create healthy and stable peri-implant tissues. An innovative volume-stable collagen matrix provides the opportunity to treat soft tissue defects around implants as an alternative to connective tissue graft.

Flapless Postextraction Socket Implant Placement, Part 2. The Effects of Bone Grafting and Provisional Restoration on Peri-implant Soft Tissue Height & Thickness - A Retrospective Study
This article presents the results of evaluating the changes in peri-implant soft tissue dimensions associated with immediate implant placement into anterior post-extraction sockets for four treatment groups: no BGPR(bone graft, no provisional restoration), PR (no bone graft, provisional restoration), BG (bone graft, no provisional restoration), and BGPR (bone graft, provisional restoration). The vertical distance of the peri-implant soft tissue was greater for grafted sites than for non-grafted ones. The facial soft tissue thickness at the gingival third was greater for grafted than for non-grafted sites and for sites with provisional restorations compared to sites without them, respectively. The net gain in soft tissue height and thickness was about 1mm. The increases in vertical and horizontal dimensions for grafted sites were between 0.5 and 1.0mm, as compared to sites with no bone graft and no provisional restoration.
