Double-arched posterior maxillary defects present a unique challenge requiring special considerations for implant replacement. This deformity is characterized by vertical bone loss from both the sinus aspect as well as the ridge. In this video, Dr. H. Ryan Kazemi demonstrates a bi-directional vertical bone augmentation technique to develop sites that not only support the implants but also improve tissue harmony for enhanced aesthetics, function, and longevity.

Thank you- This type of defect can also respond well to Ti-mesh / rhBMP2 approach as the anterior and posterior walls provide a stable foundation and support for the Ti-Mesh. I would highly consider this option if I also had a horizontaly deficiency component.

Platelet derived growth factors are now routinely utilized in reconstructive therapy. This presentation describes very detailed and evidence based guidelines for clinicians interested in enhancing their abilities in tissue engineering, especially as it relates to bone augmentation. Specifically, while successful bone augmentation requires the standard surgical parameters of space maintenance, low pressure on the grafts and tension-free flap closure, optimization of this goal requires management and enhancement of the local biological conditions with growth factors. Towards that end, platelet concentrates through the constant release of growth factors are able to promote and enhance new vascularization, provide plasma protein, normal lipidemia, as well as increased collagen and fibrin activity.

Transcrestal Sinus Floor Elevation: Most Frequently Asked Questions
Over 20 years of clinical application and research on transcrestal sinus floor elevation (TSFE) procedures using a variety of implants, graft materials and instrumentation has proven its reliability, and versatility. While improving upon the original osteotome technique they have not yet eliminated the need for a more invasive lateral approach, especially at sites with very limited (1-2 mm) sub-antral bone. Extensive analysis of the existing research and long-term clinical experience will provide the answers to the most pertinent questions surrounding the transcrestal approach including but not limited to: patient selection, clinical limitations, instrumentation, graft selection/necessity and management of membrane perforations.

Sinus Floor Elevation Via the Maxillary Premolar Extraction Socket With Immediate Implant Placement: A Case Series
When immediate implant placement is considered for teeth with close proximity to the sinus floor, apical extension of the osteotomy is significantly limited, and often a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement are techniques most often used independently or sequentially. In this care report, immediate implant placement with simultaneous osteotome sinus floor elevation is an advantageous combination of two successfully used techniques. This combined approach can significantly reduce the treatment time for implant therapy in teeth with close sinus proximity and provide the operator with the ability to place implants of desired length.
