Posterior maxillary atrophy is characterized by the loss of bone and soft tissue, complicating implant rehabilitation. Bone grafting restores volume for implant support, but soft tissue manipulation is essential for graft protection and healing. The modified palatal roll technique enhances connective tissue thickness, while the pedicled buccal pad of fat provides vascularized coverage and volume —crucial in severe atrophy to support bone regeneration and long-term stability.

Course Objectives

1. To understand the clinical indications, procedural steps, and biological principles behind the use of buccal pad of fat and palatal roll flaps in posterior maxillary reconstruction.

2. To recognize how soft tissue manipulation supports hard tissue graft protection, enhances healing outcomes, and improves long-term peri-implant tissue stability.
